PRODUCER CERTIFIED ALL NATURAL AFFIDAVIT: Producer certifies (1) that these animals have been born and raised from birth in the United States of America in a humane manner, using no antibiotics, no synthetic hormones, no steroids and no synthetic growth promotants; that to the Producer's knowledge, all animals receiving medication have been identified and removed from the contract pertaining to this agreement. (2) That vaccinations were administered subcutaneously or in the shoulder area with no more than 5cc per injection site. (3) That the following items have not been used in the production of these animals, and that all feedstuffs labels have been checked to be sure that feeds used in the production of these animals, does not contain: antibiotics, ionophores, growth  hormones, or animal by-products of any kind including but not limited to items listed by the FDA rule title 21 Section 589.2000 as explained in the Livestock Owner's Feed Certificate; and that feed used in producing these cattle does not come into contact with conventional feeds containing any of the above mentioned items; and that the proper documentation regarding feedstuffs has/will be kept and is available for a period of one year. 

LIVESTOCK OWNER'S FEED CERTIFICATE: The undersigned certifies that, to the best of his/her/its knowledge, as of the date of shipment or delivery, none of the livestock described above, will be, on such date, adulterated within the meaning of the Federal Food, Drug and Cosmetic Act (i.e., none of the cattle or other ruminants will have been fed any feed containing protein derived from mammalian tissues, e/g. meat and bone meal, as that term is defined in 21 CFR 589.2000 and none of the livestock will have an illegal level of drug residues).
